	In-House “Combined Feasibility Study and Implementation” CD-ROM Program ($18,500) () This Order Includes or Excludes the Optionally Priced Three Month Remote Consulting Services of Trust Resource Company (+$6,000) ()
	(*) A 6% Sales Tax will be Added to Orders Placed by Institutions Headquartered in Connecticut.
Kindly forward the program we are ordering hereon, and accompanying invoice, to the address below. In placing this order our institution, and the individual(s) who will utilize the Program(s) ordered hereon, acknowledge that Trust Resource Company does not practice law, that Trust Resource Company encourages legal review of appropriate materials presented in any format whether resident on the CD-ROM Program(s) we are ordering or received from Trust Resource Company via any other means, and that the CD-ROM Program(s) ordered hereon may not be shared with any other institution other than a subsidiary of our institution. Our institution also acknowledges that any decision it, or the individual(s) utilizing any input from Trust Resource Company (whether resident on the CD-ROM program being ordered or received from Trust Resource Company in any other fashion), may make relative to the conduct of a trust and asset management feasibility study analysis or plans for the potential introduction and ongoing administration of trust and asset management services are the sole decision(s) of our institution.
